Registration Requirements

This form is used for teens already enrolled for the 2024-2025 Youth Ministry school year.

If you have a new request for teen Confirmation, please contact Abby Causey via email or at (757) 937-8144.
The maximum number of form submissions has been reached. This form is currently not available.
- By registering your teen and checking the boxes below, please note that registration does not guarantee the reception of the Sacrament of Confirmation in the spring. In order for teens to receive, they must fulfill the Diocesan requirements listed below for sacramental preparation that form and engage teens in the life of the Church in the aspects of liturgy, catechesis, and parish life.
  
  If your teen is not able to commit to the Diocesan requirements and schedule, please delay preparation to another year when it is more conducive to his/her schedule for if requirements are not met, the reception of the sacrament may be delayed at the discretion of the pastor and youth ministry director.

  I would like to proceed with registration and understand teens must fulfill the requirements as follows:
  ATTEND Sunday Mass weekly and RECEIVE the Sacrament of Reconciliation prior to Confirmation. Please select this field.
- BE a registered member at Church of the Holy Family. Please select this field.
- REGISTER for Youth Ministry at Holy family (separate registration form) and attend formation sessions, youth group, and local/regional events with no more than 3 absences OR attend Catholic High School. Please select this field.
- You must contact the youth minister if you will be absent for youth formation, youth group and any youth ministry activities.
  SUBMIT a recent Sacramental Record from your teen's church of Baptism by December 20, 2023. Please select this field.
- ATTEND all five (5) In-Person Confirmation Sessions AND complete the ten (10) online modules by the scheduled due dates. The teen and one parent must attend the first session together. Please select this field.
- ATTEND the all-day Confirmation retreat in its entirety. Please select this field.
- SELECT a Catholic sponsor/godparent who meets the requirements under Canon Law and submit the diocesan Godparent Attestation form by December 20, 2023. Please select this field.
